This section requires you to input all information regarding your family details. Much of this information is a government requirement as part of your enrolment.

GROUP A - Senior management in large business organisation, government administration and defence, and qualified professionals

It is important you complete the following medical information.


Medication is not permitted to be administered by the staff at school unless a permission to dispense medication form has been completed and the medication is in the original container with the dosage and the child's name.  The medication form can be downloaded by clicking here (.pdf 294KB).

Parents/Carers are required to provide Living School with an Anaphylaxis/Asthma Plan upon commencement at school and every year for students who suffer from those conditions.  You should also let the school know as soon as you are aware of any new allergies or other medical conditions.

* If you are against vaccinations, Living School respects your rights but we will always adhere to our legal requirements to ensure we maintain our accreditation and registration status. Current legislation states if there is an outbreak, any child unvaccinated or who has not provided a copy of the Australian Immunisation Register (AIR) will be required to stay home. until the school advises.

SECTION 6 - MEDICAL TREATMENT PERMISSION

If my child should require urgent medical treatment, I authorise the school staff to seek medical attention and I agree to meet all costs.


PERMISSION TO DISPENSE PARACETAMOL (PANADOL)

If my child should require paracetamol (Panadol), I give permission for the school to dispense in keeping with directions on the packet.


SECTION 7 - SPECIAL or INDIVIDUAL NEEDS

As part of your enrolment application, you must indicate if your child has any known or emerging learning support needs.  Failure to do so may impact your child's ongoing enrolment at the school.

SECTION 8 - PUBLICITY: Taking/Use of Photographs and Digital Media

To be completed only if you DO NOT wish to have your child's images published by the school. This may have implications for newsletter and class photos.


SECTION 9 - Consent to Access Documents

I consent to the Living School gaining access to relevant information about this student, whether held by previous schools, health care professionals or other government agencies. 

I understand that the school may approach these bodies directly and obtain this information if I do not consent.

The information they request may include information related to any of the questions I have answered in this application. 


SECTION 10 - School Fees and Levies

Enrolment carries an obligation to pay all School Fees and Levies. Parents and/or Carers, who through the declaration at Section 14 make the application for enrolment, are joint and severally liable for the payment of all School Fees and Levies. 

There are a range of mechanisms available for the payment of School Fees and Levies, including
the nomination of a Third Party. Where a Third Party is nominated as the fee payer, this does not
change the liability for the Parents /Carers for any unpaid School Fees and Levies.

Parents /Carers unable to pay school fees due to genuine financial hardship are urged to approach
the Conductor (school principal) who will respond to their situation with care and sensitivity. Early
communication with the school regarding a family’s financial situation is essential.


SECTION 11 - REQUIREMENTS FOR PARENTS AND STUDENTS

Application for enrolment in the Living School means that you are making a commitment to support the philosophy, values and aims of the school and a willingness to co-operate in their implementation. Parents are reminded of the school's Code of Conduct: failure to abide by these expectations, or behaviour that may call the reputation of the school into question, could result in the loss of enrolment for your child/ren.

Your child is required to meet the school’s high expectations basically described as a love of learning and being kind.  To detail this more fully, our expectations of your child are clear: 

  1. behaviour and self discipline including adherence to anti-bullying policies 
  2. compulsory attendance at school
  3. application to course work and study
  4. participation in school activities 
  5. participation in the life of the school
  6. dress codes, e.g. non-offensive and safely wear as/when directed

Your co-operation is essential to assist your child to attain these expectations. Parents are expected to participate in the total life of the school through events and activities.

On acceptance of enrolment, you agree to support all the Living School’s policies and procedures.


SECTION 12 - DECLARATION

I/We have read and agree to the responsibilities stated above in ‘SECTION 10 School Fees and Levies’ and ‘SECTION 11 Requirements for Parents and Students’ and apply for enrolment of my/our child subject to these expectations. Where I have given personal information about people other than myself or my child(ren) I have done so with their authorisation.

I/We declare that the information provided in this Enrolment Application is, to the best of my/our knowledge and belief, accurate and complete. I/We recognise that, should statements in this application later provide to be false or misleading, any decision made as a result of this application may be reversed.

I/We agree, if the enrolment is accepted, we will meet our obligations as listed in the Application and outlined in policies of the school.

SECTION 14 - PRIVACY STANDARD COLLECTION NOTICE

1. The School collects personal information, including sensitive information about students and parents or guardians before and during the course of a student’s enrolment at the School. The primary purpose of collecting this information is to enable the School to provide schooling to the student and enable the student to take part in all the activities of the School.

2. Some of the information we collect is to satisfy the School’s legal obligations, particularly to enable the School to discharge its duty of care.

3. Laws governing or relating to the operation of schools require certain information to be collected and disclosed. These include relevant Education Acts, Public Health and Child Protection laws.

4. Health information about students is sensitive information within the terms of the Australian Privacy Principles under the Privacy Act. We may ask you to provide medical reports about students from time to time.

5. If we cannot obtain the information referred to above, we may be unable to enrol or continue the enrolment of your child.

6. The School, from time to time, discloses personal and sensitive information to others for administrative and educational purposes, including to facilitate the transfer of a student to another school. This includes to other schools; government departments or agencies such as the Association of Independent Schools, NSW Education Standards Authority (NESA) NSW Department of Education, NSW Education Standards Authority, the Australian Curriculum, Assessment and Reporting Authority (ACARA); medical practitioners and people providing services to the School, including specialist visiting teachers, (sports) coaches, volunteers, counsellors and professional service agencies.

7. Personal information may be used for the purpose of publishing certain school information relating to the general circumstances of parents and students on the MySchool website, in accordance with Australian Government requirements.

8. Personal information collected from students is regularly disclosed to their parents or guardians.

9. The School may utilise external providers to deliver certain services including ‘cloud’ data storage to the School and its staff and students. The School may store personal information in the ‘cloud’ which may mean that it resides on servers that are located outside Australia.

10. The School has a Policy that sets out how parents or students may seek access to personal information collected about them. However, there will be occasions when access is denied. Such occasions would include where access would have an unreasonable impact on the privacy of others, where access may result in a breach of the School’s duty of care to the student, or where students have provided information in confidence.

11. The School from time to time may engage in fundraising activities. Information received from you may be used to make an appeal to you. Such information may also be disclosed to organisations that assist in the School’s fundraising activities solely for that purpose. We will not disclose your personal information to third parties for their own marketing purposes without your consent.

12. On occasions information such as academic and sporting achievements, student activities and school related news is published in School newsletters, magazines or other publications and on the School website. Photographs of student activities such as academic or sporting events, school camps and excursions may be taken for publication in newsletters and magazines and on our intranet.

13. The School may include your contact details in a class list and/or School directory.

14. If you provide the School with the personal information of others, such as doctors or emergency contacts, we encourage you to inform them that you are disclosing that information to the School and why, that they can access that information if they wish and that the School does not usually disclose this information to third parties.
